Plumbing supplier of your choice. You need a brass tee, two 5/8" barbs, and a 5/16" barb. In a pinch you also could use a couple of 3/8 NPT pipe nipples (5/8" actual OD) and a 1/8 NPT nipple (~3/8" OD).

Is it corroded or is the solder joint leaking? If it's just the joint leaking a plumbing shop should be able to re-solder it for you.
I haven't taken it out, It's just wet around it.

They corrode through pretty easily.

I have brass ones. (Bleed adaptors ). Same price as the steel one shown (we have those too). The OEM aluminum ones very often rot through at the tee joint, and/or get so corroded the hose won't seal.

Did not know the the OEM was aluminum. I thought it was brass like yours. Goes to show you I have never had that connection apart on my car. (YET)

Careful when changing it out that you don't damage your hot water valve - or just take this opportunity to replace that as well. I am firmly convinced that careless handling of the heater hoses directly connected to it will lead to failure of the valve.
Very Bad Things can happen if that valve lets go - ask me or about 20 other owners on here how we know :)
