I'm wondering if they aren't tight enough it could cause play which would then allow a good jolt to pull them through??? I've had the Spax for nearly two years now and will inspect them soon to verify the washers still look good.
    You should not bottom out the nut. Just tight enough so the bushing expands to the edge of or slightly bigger than the washer provided.
